In the field of home and community care, KHS is dedicated to providing timely hospital liaison and discharge planning for clients with decreased functional mobility and difficulty in activities of daily living. Occupational therapists complete home accessibility assessments and provide recommendations and case co-ordination regarding medical equipment, home renovations, rehabilitation, and personal care support.
